Key Responsibilities:

Patient-Centered Care:
  • Conduct comprehensive assessments to determine hospice needs and develop personalized care plans.
  • Utilize the nursing process to provide compassionate and professional care, incorporating palliative nursing actions.
  • Collaborate with patients and families to establish achievable goals and promote self-sufficiency and comfort care.
  • Act as a Case Manager, coordinating patient care and ensuring continuity of services.

Effective Communication:
  • Maintain accurate clinical documentation and communicate patient needs and changes to the interdisciplinary team and physicians.
  • Foster open communication with community health professionals to coordinate care plans effectively.
  • Educate patients and families on self-care techniques, medication management, and other relevant instructions.

Additional Duties:
  • Participate in on-call duties as outlined in the policy.
  • Supervise ancillary personnel and delegate responsibilities as needed.
  • Engage in personal and professional development activities, including continuing education and quality improvement initiatives.

Qualifications:
  • Current licensure as a Registered Nurse in the state of California.
  • Graduate of a National League for Nursing accredited school of nursing.
  • CPR certification and a valid driver's license with insurance.
  • Minimum of one year of nursing experience within the last three years, preferably in public health, home care, or hospice nursing.
  • Strong organizational and communication skills with the ability to work independently.
Pay: $55 - $60 per hour based on experience; 40 hours guaranteed
