Key Responsibilities:
Patient-Centered Care:
- Conduct comprehensive assessments to determine hospice needs and develop personalized care plans.
- Utilize the nursing process to provide compassionate and professional care, incorporating palliative nursing actions.
- Collaborate with patients and families to establish achievable goals and promote self-sufficiency and comfort care.
- Act as a Case Manager, coordinating patient care and ensuring continuity of services.
Effective Communication:
- Maintain accurate clinical documentation and communicate patient needs and changes to the interdisciplinary team and physicians.
- Foster open communication with community health professionals to coordinate care plans effectively.
- Educate patients and families on self-care techniques, medication management, and other relevant instructions.
Additional Duties:
- Participate in on-call duties as outlined in the policy.
- Supervise ancillary personnel and delegate responsibilities as needed.
- Engage in personal and professional development activities, including continuing education and quality improvement initiatives.
Qualifications:
- Current licensure as a Registered Nurse in the state of California.
- Graduate of a National League for Nursing accredited school of nursing.
- CPR certification and a valid driver's license with insurance.
- Minimum of one year of nursing experience within the last three years, preferably in public health, home care, or hospice nursing.
- Strong organizational and communication skills with the ability to work independently.
