Job Description
We are looking to hire a Physical Therapist to join our team in providing excellent healthcare to our client’s patients. This position will be responsible for working with patients to improve their physical health and manage their pain based on treatment plans.
Responsibilities:
- Provides quality patient care and family training through evaluation, treatment planning, and the application of physical therapy procedures.
- Acts as a rehabilitation consultant for patients needing efficient treatment intervention.
- Screens and evaluates patients to determine need for age specific tools, intervention, and referral initiation if needed.
- Establishes goals and treatment plans based on evaluation data and in coordination with goals and plans of patient, family and other care team members.
- Interprets data to plan and implement patient treatment programs which are safe and effective.
- Modifies treatment program according to patient needs and progress, terminates treatment appropriately, and develops appropriate discharge plans.
- Completes evaluations, progress notes, and discharge summaries.
- Establishes and maintains therapeutic relationships with patient, family, and caregivers.
- Perform other duties as assigned.
Basic Qualifications:
- Minimum one (1) year of full-time experience as a Physical Therapist is required.
- Graduation from an accredited Physical Therapy program is required.
- Current California Physical Therapist license is required.
- Current CPR and BLS certifications are required.
- Basic computer skills are required.
- Customer service, verbal and written, and detail-oriented skills are required.
