MDS Coordinator

Job Summary: The MDS (Minimum Data Set) Coordinator plays a pivotal role in ensuring the accurate and timely completion of resident assessments in compliance with regulatory requirements. This position requires a keen attention to detail, strong organizational skills, and a deep understanding of nursing care plans and regulatory guidelines. The MDS Coordinator collaborates with interdisciplinary teams to assess, plan, implement, and evaluate resident care plans, striving for optimal health outcomes and regulatory compliance.

Responsibilities:
  1. Conduct comprehensive resident assessments in accordance with regulatory guidelines and facility policies.
  2. Coordinate and oversee the completion of the Minimum Data Set (MDS) for each resident, ensuring accuracy and completeness.
  3. Collaborate with interdisciplinary teams, including nursing staff, physicians, therapists, and social workers, to gather information for resident assessments.
  4. Analyze assessment data to develop individualized care plans that address the physical, psychosocial, and emotional needs of residents.
  5. Maintain up-to-date knowledge of regulatory requirements related to MDS assessments and ensure compliance with state and federal regulations.
  6. Monitor and track changes in resident conditions, updating care plans as needed to reflect changes in health status or treatment plans.
  7. Serve as a resource for staff regarding MDS coding, documentation requirements, and regulatory compliance.
  8. Participate in quality improvement initiatives and regulatory surveys, providing documentation and support as needed.
  9. Provide education and training to staff on MDS assessment processes, documentation requirements, and regulatory updates.
  10. Maintain confidentiality of resident information and adhere to HIPAA guidelines in all aspects of job performance.
Qualifications:
  1. Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid nursing license.
  2. Minimum of 2 years of clinical experience in a long-term care or skilled nursing facility.
  3. Certification in MDS assessment preferred.
  4. Strong understanding of nursing care planning processes and regulatory requirements.
  5. Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
  6. Proficiency in computer skills, including experience with electronic health records (EHR) and MDS software.
  7. Attention to detail and accuracy in documentation and data entry.
  8. Ability to prioritize tasks and manage time effectively in a fast-paced environment.
  9. Commitment to maintaining compliance with state and federal regulations and upholding ethical standards in resident care.
  10. Continuous commitment to professional development and staying updated on changes in regulatory requirements and best practices in long-term care.
Working Conditions:
  • The MDS Coordinator primarily works in an office setting within a long-term care or skilled nursing facility.
  • May require occasional travel for training or professional development opportunities.
  • Must be able to work flexible hours to accommodate resident assessment needs and regulatory deadlines.
