Location: Anaheim, CA

Position Overview: We are seeking a dedicated and empathetic Case Manager to join our dynamic team. The Case Manager will play a crucial role in our mission by providing personalized support and guidance to individuals and families facing homelessness. This position offers the opportunity to make a meaningful impact on the lives of those in need and contribute to our ongoing efforts to combat homelessness.

Key Responsibilities:
  • Conduct comprehensive assessments to understand the unique needs and challenges of clients.
  • Develop and implement individualized case plans in collaboration with clients, focusing on housing stability, employment, healthcare, and other essential services.
  • Provide ongoing support and advocacy to clients, assisting them in accessing resources and services such as housing assistance, mental health counseling, substance abuse treatment, and job training.
  • Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with relevant regulations.
  • Monitor progress toward goals and make necessary adjustments to case plans as needed.
  • Collaborate with community partners and agencies to coordinate services and resources for clients.
  • Participate in regular team meetings, case conferences, and training opportunities to enhance professional growth and knowledge.

Qualifications:
  • 2 years of experience and/or Bachelor's degree in social work, psychology, counseling, or a related field.
  • Previous experience in case management, preferably in the nonprofit sector or within homeless services.
  • Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds.
  • Excellent communication skills, both written and verbal.
  • Empathy, patience, and a genuine desire to help others.
  • Knowledge of local resources and services related to homelessness and social services.
  • Ability to work independently and as part of a collaborative team.
  • Cultural competency and a commitment to diversity and inclusion.
  • Valid CA drivers license
  • At least 1 years of experience working with at risk/homeless individuals (preferred)
  • Must be familiar with VI-SPDAT.
Preferred:
  • Bilingual in English and Spanish.
  • Experience in the area of non-profit housing and/or housing for people with disabilities and chronic health conditions
  • Proficiency in Google Suite
