Essential Qualifications:
- A Bachelor’s Degree in Nursing or completion of an accredited LVN program.
- At least two years of experience in Quality, Health Planning, Case Management, or Population Health.
- Familiarity with electronic health records and a history of working on QI projects.
- Current LVN License and BLS Certification.
Preferred Qualifications:
- Advanced degrees in Nursing, Public Health, or related fields.
- Extensive experience in quality, planning, case management, or population health.
- Proficiency in languages other than English, especially Spanish, and experience with diverse populations.
- Familiarity with healthcare platforms like Allscripts, FollowMyHealth, and others.

While primarily in an office, there may be times you'll engage in more active duties. Safety is paramount, especially when handling biohazardous materials or encountering potential exposure to blood-borne pathogens.
